I agree that my dog is up to date with all vaccines, friendly to both humans and other canines, does not exhibit aggressive behavior, will be on-leash at all times during the event, and is not prone to excessive barking or growling.
I understand I may be asked to leave the main auditorium or the event premise at the discretion of DOGOPOLIS, LLC ("DOGOPOLIS") event staff if any of the above is violated.
I understand that attendance of this event is not without risk to myself, my guests who may attend, or to my dog. I hereby agree to indemnify and hold harmless DOGOPOLIS, BRAVO!PUP, MINE THE MOVIE, LLC, DOGPILE DOGS, LLC, SCRAP AND TAFFY PRODUCTIONS, LLC ("the parties"), and its officers, directors, instructors, employees and/or representatives of any and all claims, or claims by accompanying guests of mine of injury, expense, costs or damages to myself or my dog. In addition, I agree that I will defend and indemnify the parties for any injury, expense, costs or damages to any dog or to third parties arising out of my own actions or the actions of my dog. I have read the above-stated provisions, and agree to accept those responsibilities.
